New Patients

You can register as a patient at the practice wherever you live, but you won’t be eligible for home visits unless you live within the Practice Boundary.

If you have a medical card, please complete the change of address section, not forgetting postcode and telephone number and bring it along to reception.

If you are unable to find your medical card, you will need to collect a registration form (GMS1) from reception or you can complete the form below. Please fill this form in carefully, remembering to include all postcodes, telephone numbers and date of birth. Alternatively you can download and complete the form here.

You will need to visit the practice with two forms of ID to enable us to process your registration.
