To update details like your name, address, phone numbers or email address:
- in your NHS account (through the NHS website or NHS App)
- in your SystmOnline account
- use our accuRx service
- phone us on 01761 432121 or visit the surgery from 10am to 6pm
- email bswicb.hopehouse@nhs.net
Change of address
If you new address is outside of our catchment area it may be possible for you to stay registered with our surgery, but you will not be eligible for home visits.
Changing your phone number or email address
You will need to show us identification before we can change your phone number or email address.
Changing your name
You will need to show us proof that you have changed your name. This could be a marriage or civil partnership certificate, deed poll or birth certificate.
